There are 3 parts to the application process. You will have to send in a copy of your latest tax return as well as a letter verifying your financial need and complete the application.
Tax form - your latest IRS 1040 tax return needs to be submitted. If you don’t file taxes, we need to see your approval form for any state assistance or disability you receive (COMPASS form). We can NOT use the SSI letters for this.
Referral Letter verifying finacial need - the letter cannot come from relative and must be from a social services agency, church, or similar non-profit organization. Any volunteer work that you do would qualify for this. The letter needs to state that you are in financial need.
